What is Healthy Yolo?
In 2013 the Yolo County Community Health Branch staff initiated a Community Health Assessment that led to the identification of three priority areas: Mental Health, Chronic Disease Prevention, and Healthy Aging. Work groups, comprised of County and community partners, convened around each of these priorities and developed the Community Health Improvement Plan (CHIP). Keeping in mind the guiding principles of health equity, social determinants of health, life course perspective, and upstream investments, the work groups are working towards collective impact.
