Key Activities and Accomplishments of Healthy Yolo
In 2013 the Yolo County Community Health Branch staff initiated a Community Health Assessment that led to the identification of three priority areas: Mental Health, Chronic Disease Prevention, and Healthy Aging. Work groups, comprised of County and community partners, convened around each of these priorities and developed the Community Health Improvement Plan (CHIP). The CHIP has since been used by the community and the Healthy Yolo work groups to implement a collective impact approach to guide efforts to improve health outcomes through practices that reinforce the guiding principles of health equity, the social determinants of health, a life course perspective, and upstream investments.
