YCHIP

The Yolo County Healthcare for Indigents Program (YCHIP) was designed to provide medically necessary health care for qualified residents of Yolo County. Eligibility for the program is determined on the basis of residency and income. When it is determined that you qualify for the program, it will also be determined if you will have a co-payment for your care. The co-payment is based on your income and your family size. If you have a co-payment, you must pay your co-payment at the point of service.

To apply for YCHIP certification, you will need to come to the YCHIP office for an interview. You can download English and Spanish versions of the YCHIP application below, and have them filled out for your interview, however, forms cannot be dropped off or mailed in. YCHIP will not be responsible for medical or dental care until your application is approved. Please carry your approved YCHIP verification with you when you seek medical or dental care.

You may apply for YCHIP certification at Peterson or Salud Clinics Only.  No new applications will be processed at the Davis Clinic.  Below are the three primary care clinics for medical service.  Details for Winters Clinic will be added soon.  YCHIP members may visit any of our approved clinics for medical care.

Persons applying for YCHIP should try to arrive at one of our offices and sign in at least 30 minutes before closing time. Sign-in sheets are removed each day when there is not enough time to see any additional applicants. If you arrive after the sign-in sheet is removed, you may not be seen and may need to return the next business day.
