(Woodland, CA) – The Yolo County Health Department, through the Healthy Yolo campaign, is working on the Healthy Yolo 2014 Community Health Assessment. The Community Health Assessment is the process used to examine the health of Yolo County and to identify areas where organizations, policy-makers and residents can work together to improve our community’s health. Gathering and reviewing statistical information, such as the rate of deaths from heart disease and prevalence of risk factors that affect health outcomes, is complete. The next step is to share this information with residents and seek their input regarding the health of our community.

The Community Health Assessment is available online ( for community members to view and comment on. Healthy Yolo has also made available seven regional reports.

A series of community forums will be held in Yolo County during the month of April. These forums are designed to share current county and community-specific health data. Attendees will have an opportunity to share their views about health issues in their communities. Attendees will also be asked to identify strategic issues, which are those fundamental policy choices or critical challenges that must be addressed in order for a community to improve the health and well-being of its residents. All forums are open to the public and anyone may attend these meetings.
